Click on the links below to print an enrollment form. You will need to read over the appropriate school enrollment flyer with your child to select the classes your child would like to participate in. The enrollment flyer also includes important information for your child's participation in the program. Please read it carefully!

Once you have completed and signed the enrollment form you can turn it in at any one of the enrollment dates or to your child's home campus.Please contact the After School Action Program office if you have questions 943-5000 ext.6925.
