the information you need to enroll your child at BMS, please download
and print registration forms below.
To
view and print these forms, you will need Adobe Acrobat Reader software. If you do not have Acrobat Reader, it is available for free download at the
Adobe
web site.
Benold Middle School Registration for 2009-2010
To expedite the registration process, please
bring the following items with you when you come to Benold to
register your child:
- The forms from the appropriate registration packet, filled
out as completely as possible
- Driver's license or photo ID for parent/guardian
registering student
- Student's social security card
- Student's birth certificate
- Student's current immunization record signed by a physician
(Click
here for new immunization requirements for 2009-2010.)
- Proof of residency in the Benold attendance zone, such as a
utility bill or rental contract agreement
- Copies of your child's school records, such as most recent
report card or standardized test scores (these are not necessary,
but are very helpful if you have them)
