MEDICATIONS AT SCHOOL

Dear Parents,

    If you'd like for your kids to have any medications here at school we need your permission to give them. Bellow this message you will find the official GISD Medication Permission form for that. Please read it carefully before filling it out.

  •   Click the link below, print it out, fill it out, and then either turn it in by hand or fax it in @ 943-5159.
     

  •  Remember only you can provide these medications for your child. Please do not send them with your child, they must be brought to school by you.
     

  •  The medications must come in the original prescription bottles with the pharmacy label in tact or in the bottles in which you purchased them in for OTC medications.
     

  •  Please call me with questions or concerns. Feel free to keep me informed about your child's health status as well, so that I can help keep them and others  well and healthy. Thank you!
