If you would like to refer your child to SAIL for an evaluation, contact a campus counselor or campus SAIL team coordinator for a SAIL referral form.  Fill out the form and return it to them.  A SAIL coordinator will contact you to seek your input. 

All referrals are kept strictly confidential.
