The following forms must be completed and submitted for approval by one of the Staff Athletic Trainers before you can participate in athletics at Georgetown High School.  All documents combined make up the entire physical packet that will be placed in the student-athletes medical folder kept in the athletic training room.  All physicals that are for the 2008-09 school year, must be dated after May 1, 2008 and be on the UIL Pre-participation Physical Evaluation and Medical History Form.

All paperwork must be submitted and reviewed by one of the Staff Athletic Trainers before the student-athlete is cleared for participation in any practice, scrimmage, and/or game.

GHS DRUG TESTING FORMS

Additionally, GISD participates in random drug testing to all students that participate in any UIL activity, including athletics.  Please complete the form that applies to you and submit them to the head coach of the sport you participate in.  To review the drug testing policy for GISD please click here.  If you have any questions regarding the drug testing in GISD please contact Lana Weeks at (512) 943-5000 ext 7550.
