Decision Matrix

Home Questions to Ask Self Determination Graduation Pathways Resources Parent Checklist Decision Matrix

This Matrix represents the typical developmental skills of any (PPCD through 18+) student within the areas of social / recreation / leisure, living skills, classroom skills (life – long learning) and skills towards employment.  Use this matrix to identify where your child is currently functioning  within each of the four areas.

Start with the Idea Generators for social / recreation / leisure.

1.       Compare the first item in each performance level within that vertical column.   Which one sounds most like your child – full Independence, Functional Independence, Supported Independence, or Participation?

2.       Check the box that best currently describes your child.

3.       Continue to work your way down this column comparing the 2nd item in each performance level, then the 3rd, etc.

4.       Using the same process please identify where your child’s skills currently are within the remaining three life domains (Adult Living, Lifelong Learning, and Employment).

5.       Once completed, please return to your child’s Case Manager.
