This form is to be completed and submitted to the campus secretary for each and every student club or charter participating in any fundraising. For new clubs/charters, the form should be submitted within 14 days of the formation of the club/charter. At the beginning of each school year, a new form is required for all active clubs/charters. The campus secretary will forward scanned copies of each form to the Business Servces and retain the originals on campus.
