NJROTC Cadet Required Admin Forms
The three forms below are the most important forms that allow cadets to be in the Navy JROTC program for our Annual Military Inspection Audits, to participate in physical fitness, and conduct the Navy Physical Fitness Test in order to earn the PRT Ribbon, for Varsity Cadets to compete at different Drill Meets, Area Ten State Finals and Navy JROTC Nationals; and finally to be able to attend Summer Leadership Camps.
We recommend that cadets and parents do the forms in the following order:

Health Risk Screening Questionnaire (If you answer "Yes" to any question in this form, you need a doctor's signature)

Health Risk Screening Questionnaire
At the bottom of Page 1, Parents and Cadets must sign and date the form. IF ANY QUESTION IS ANSWERED "YES"; then the form must be signed by a doctor and date.

Preparticipation Physical Evaluation
Current Preparation Physical Evaluation consists of two pages; page 1 is the student's Medical History and page 2 is the Physical Examination.It is critical that the Parent and Cadet sign and date at the bottom of Page 1. It is critical that a doctor makes a selection of the Clearance section and signs it at the bottom of Page 2.

Please make every effort to fill in all the information required on this form, including insurance information. Make sure Parents sign at the bottom of page 2 and include a good phone number to contact.
