Mental Health Services & Parental Consent

 

Log into Skyward Family Access.  (instructions for login here)

Select the Online Forms button, then click the link that says Fill Out Form next to Mental-Health Related Services & Parental Consent. (Note: If you have previously submitted this form, click the link that says View next to the form instead.)

The Mental-Health Related Services & Parental Consent form will open. Click Next.

Click on the View Full Screen button to view the form clearly.

A full screen view of the form will open.  Please read the document carefully and when ready, select Yes or No in the consent section of the form.

When you are done, click the button that says “Complete Step 1 and move to Step 2”. (Note: If you are in full screen mode the button will be at the top of the screen.  If you are not in full screen mode then the button will be at the bottom of the screen.)

On the last page, click the button that says Submit Mental-Health Related Services & Parental Consent.  Your form will be submitted.
